This survey provides feedback in regards to your development process so that you and our other clients may continue to experience success.  Leadership Resources uses this survey to gauge how we are succeeding in fulfilling our commitment to you and to identify additional opportunities for improving the development process. 

Information you will need:

• Name of development process (ELD, EPP, ESM, etc)
• Program Code
• Name of Facilitator
• Collection Point (Mid-Point, End of Program, End of Process)

All of this information can be found on your program schedule that was provided at the start of your program sessions.

We challenge you to be real with your answers to the questions - we are continuously looking for ways to improve and your feedback is vital to our development.  We believe in being transparent and open with everyone we work with, so it is important that you provide honest and genuine responses.  Keeping that in mind, be aware that your survey responses may be shared with the Leadership Resources team and your mentor. Additionally, if your organization has sponsored your participation in this development process, your answers may be shared with your direct supervisor and/or the economic decision maker from your organization.
